CFSS Consultation Services
What is CFSS? CFSS is a participant-controlled program in Minnesota designed to offer more choice, control, and flexibility than the legacy Personal Care Assistance (PCA) and Consumer Support Grant (CSG) programs. As of October 1, 2024, CFSS has begun replacing these legacy programs, allowing participants to hire family members (including spouses and parents of minors) and purchase goods that aid in their independence.
Our Services: Expert Consultation & Support
Safe Bridge LLC provides comprehensive support throughout your CFSS journey. Our services are authorized for six sessions per year (up to $600 annually), ensuring you have support from your initial orientation through your annual reassessment.
Service Set-up & Orientation: We provide a thorough overview of CFSS, explaining your rights, responsibilities, and the various ways the program can meet your needs.
Service Delivery Plan (SDP) Development: We assist you in writing your Individual Service Delivery Plan (DHS-6893P), ensuring it captures your goals and meets all state requirements for approval.
Model Selection Guidance: We help you weigh the pros and cons of the Agency Provider Model versus the Budget Model so you can decide which level of control is right for you.
Ongoing Support: Our team responds to your questions throughout the year, assisting with plan revisions if your health needs change.
Quality Assurance: For those using the Budget Model or hiring family members, we conduct required semi-annual reviews to ensure your services remain safe and effective.
